Low Back Pain at the Desk: Why Sitting Hurts—and What Actually Helps

Low back pain is one of the most common reasons people miss work, and for many, the culprit isn’t heavy lifting—it’s sitting at a desk for eight hours a day. While sitting may feel harmless, prolonged, uninterrupted sitting places unique stresses on the spine and surrounding tissues that can quietly build into pain over time.

Why Sitting All Day Causes Low Back Pain

When you sit for long periods, especially with a slouched posture, several things happen:

  • Spinal compression increases. Sitting places more pressure on the lumbar discs than standing, particularly when the lower back is rounded. Over time, this can contribute to stiffness and discomfort.
  • Muscles switch off—or overwork. The glute muscles and deep core stabilizers tend to become less active, while the hip flexors and lower back muscles may stay shortened or tense.
  • Reduced circulation and movement. Joints and discs rely on movement to circulate nutrients and remove waste. Staying still for hours deprives the spine of this healthy “motion nutrition.”

The result? Stiffness when you stand up, aching by the end of the day, and pain that can linger into evenings and weekends.

The Good News: Movement Is Powerful Medicine

One of the most effective ways to manage and prevent desk-related low back pain is regular movement throughout the day—not just one workout before or after work.

A well-known Cochrane Review (Hayden et al., 2021) found that exercise is effective in reducing pain and improving function in people with chronic low back pain (1)  Importantly, the benefits were linked to consistent movement, not rest. This supports what clinicians see daily: backs tend to feel better when they move more often.

Simple Exercises and Stretches You Can Do at Work

You don’t need gym clothes or a yoga mat. These gentle movements can be done in work clothes, right at your desk.

  1. Seated Pelvic Tilts (1–2 minutes)
    Sit tall and gently rock your pelvis forward and backward, alternating between a small arch and a slight tuck.
    Why it helps: Restores motion to the lumbar spine and reduces stiffness.
  2. Standing Back Extensions (5–10 reps)
    Stand up, place your hands on your hips, and gently lean backward, then return to neutral.
    Why it helps: Counters prolonged forward-bent sitting posture.
  3. Hip Flexor Stretch (30 seconds each side)
    Step one foot back into a short lunge, gently tucking the pelvis under until you feel a stretch at the front of the hip.
    Why it helps: Sitting shortens hip flexors, which can pull on the low back.
  4. Desk Break Walks (2–5 minutes)
    Set a timer every 30–60 minutes to stand up and walk, even briefly.
    Why it helps: Promotes circulation, joint lubrication, and muscle activation.

Rethinking the Workday

The goal isn’t perfect posture or eliminating sitting entirely—it’s breaking up long periods of stillness. Sit, stand, move, repeat. Small, frequent movement snacks throughout the day add up to meaningful relief.
